Identity cards are not issued for our dental and hearing aid plan.
DENTAL/HEARING AIDS
The Plan will pay up to $2,500.00 per family for dental work done and/or hearing aids purchased within the calendar year.
Dental
Payment will be made on presentation of the following:
An ORIGINAL PAID RECEIPT and an ORGINAL STANDARD DENTAL CLAIM FORM which shows the date of service, amount of payment and name of member (or dependent). If there is another plan involved with a claim, members must provide copies of this coverage along with the above originals.
Note: Unnecessary or cosmetic dentistry, including bleaching and veneers, are not covered by this Plan expect with the consent of the Plan Trustees.
Any dental expenses resulting from an accidental injury are covered under your extended heath plan, please check your Pacific Blue Cross brochure for details.
Hearing Aids
Payment will be made on presentation of the following:
An ORIGINAL ITEMIZED PAID RECIEPT indicating who the hearing aid was purchased for and the method of payment.
Note: Hearing aids for dependent children under twenty-one years of age are covered under your extended health plan, please check your Pacific Blue Cross brochure for details.
**Please be advised that the Plan Trustees have the ability to pro-rate coverage. Members with less than 4 months of coverage in the calendar year will be pro-rated to 1/12 of the dental limit for each month of coverage.
**The last day of payment for dental and hearing aid claims will be March 31st of the following year.
Forward your ORIGINAL PAID RECEIPT and STANDARD DENTAL CLAIM FORM for dental or your ORIGINAL ITEMIZED PAID RECEIPT for hearing aids to:
